As overdose deaths begin to fall in the US and Canada, Vangelov et al.’s research has linked the decline to changes in the illicit fentanyl supply. In our response, we take a closer look at that argument and ask what gets missed when supply disruption is treated as a policy success.
Drawing on lived and living experience, we challenge the idea that reducing fentanyl availability offers a straightforward route to safer drug markets. We argue instead for a response centred on harm reduction, meaningful involvement of people who use drugs in policy development, decriminalisation and legally regulated safer supply.
