The deep and severe loss of global health funding has left lifesaving harm reduction services, HIV and hepatitis care, and community-led programming hanging by a thread. This guide underscores both what must be protected and what must be fought for, because without deliberate, informed, and sustained community engagement in GC8 processes, the hard-won gains of people who use drugs risk being erased. In this new funding landscape, where resources are shrinking and the push for integration is accelerating, people who use drugs must be at the table, not as an afterthought, but as leaders shaping the decisions that will determine whether our communities survive or are left behind.
